  Observation. Licensing observed that under the bathroom sink there were two cans of hairspray and one container of mouthwash. All products stated that they should be kept out of reach of children.

  Corrective action. Ensure that children do not have access to any object or substance that may be hazardous to their health or safety.

  Observation. Licensing reviewed five of seven children's records. Licensing observed that one child was missing medical service plan number, and another child was missing dates for record of immunization status.

  Corrective action. Ensure that, for each child, there is a medical service plan number and a record of immunization status.

  Observation. Licensing reviewed five of seven children's records. One child was missing the name and telephone number of a medical practitioner.

  Corrective action. Ensure that, for each child, there is the name and telephone number of a medical practitioner.

  Observation. Licensing reviewed five of seven children's records. Two children's records were missing a signed copy of the repayment agreement.
