• For the purpose of paragraph (a) of the definition of 'care' in section 1 of the Act, the following programs are prescribed: Family Child Care, being a program in which the licensee personally provides care, within the licensee's personal residence, to no more than 7 children;

  Observation. Regarding intake dated April 21, 2026. Through the investigation process Licensing determined that the Licensee, by admission, was not present and did not personally provide direct care to children, on several occasions.

  Corrective action. Ensure the Licensee personally provides care, within the licensee's personal residence, to no more than 7 children. Action/s required by Licensee. Licensee to submit a corrective action plan to Licensing specific to the contravention.

  3030 - CCLR 2(1)(e)(ii)
• A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

  Observation. Regarding intake dated April 21, 2026. Through the investigation process, Licensing determined that the Licensee, by admission, failed to submit incident reports documenting multiple facility closures related to service delivery disruptions due to the Licensee's illness.

  Corrective action. Ensure a licenses notifies the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s required by Licensee. Licensee to submit a a corrective action plan to Licensing specific to the contravention.

• A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

  Observation. Licensing observed that a log of minor incidents had not been kept for January and February 2026.

  Corrective action. Ensure a licensee must current records of each of the following: a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

• A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

  Observation. Licensing audited 6 children's files. Two were missing medical services plan numbers and immunization status.

  Corrective action. Ensure current records must be kept for each child in care showing medical services plan number and immunization status.

• A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are safely constructed, free from hazards and in good repair.

  Observation. Licensing observed sharp corner of unhinged plexi-glass accessible to children in the outdoor play space.

  Corrective action. Ensure to repair the plexiglass to mitigate the risk of health and safety of children in care.

  3000 - CCLR 16(4)(b)
• A licensee must keep, for each child, a record showing the following information: (k) the date on which the child stops attending the community care facility.

  Observation. Licensing observed one child's registration form had no record showing date withdrawn.

  Corrective action. Ensure to provide date withdrawn on child's registration form.

  3070 - CCLR 57(2)(k)
• A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

  Observation. Licensing observed log of minor accidents/illnesses/unexpected events involving children had not been documented since February 2024. Licensee shared several minor incidents with Licensing that should have been recorded in the log.

  Corrective action. Ensure to log minor accidents/illness's/unexpected events.

  Observation. Licensing observed Licensee had not kept records of fire drills for 2024.

  Corrective action. Ensure to provide Licensing with a record of fire drills for 2024.

  3020 - CCLR 56(1)(c)
• A licensee must ensure that children have at all times immediate access to an employee who holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C,

  Observation. Licensing observed the Licensee's first aid certificate was expired.

  Corrective action. Ensure to recertify first aid certificate.

  Observation. Licensing observed a child's file audited had no record showing person's not permitted.

• A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees

  Observation. Licensing observed the facility fire extinguisher had not been maintained annually.

  Corrective action. Ensure to maintence fire extinguisher annually.

  Observation. Licensing observed that there is no log being kept of all inspections, maintenance & repairs including outdoor play space checklist.

  Corrective action. Ensure that daily check is being recorded of all inspections, maintenance & repairs.

  Observation. Licensing observed that the record of minor incidents/illness/unexpected events involving children that did not require medical attention & were not reportable incidents was not kept current.

  Corrective action. Ensure that licensee must keep/obtain current record of minor incidents/illness/unexpected events involving children that did not require medical attention & were not reportable incidents are kept current.

  Observation. Licensing reviewed two staff files. One of the files was missing two of the records required under section 19(1) character and skill requirements.

  Corrective action. Ensure that facility obtains records required under section 19(1). 2x character and skill requirements.

  Observation. Licensing observed monthly fire drill log. Fire drill was not conducted for the month of May, 2022.

  Corrective action. Ensure licensee must keep a current record re: implementation of monthly fire drill system and yearly emergency plan practices.

  Observation. Licensing audited three of three staff files. Two files were missing copies of certification.

  Corrective action. Ensure licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: copies of diplomas/certificates/evidence/training/skills obtained for employees.

  Observation. Licensing audited three of three staff files. Two files were missing character references.

  Corrective action. Ensure licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: character references obtained for employees.

  Observation. Licensing audited three of three staff files. One file was missing a criminal record check.

  Corrective action. Ensure licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: all criminal record check results & character references must be kept for those ordinarily present for the entire time they remain ordinarily present.
