    Observation. Regarding intake dated April 21, 2026. Through the investigation process Licensing determined that the Licensee, by admission, was not present and did not personally provide direct care to children, on several occasions.

    Corrective action. Ensure the Licensee personally provides care, within the licensee's personal residence, to no more than 7 children. Action/s required by Licensee. Licensee to submit a corrective action plan to Licensing specific to the contravention.

  • A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

    Observation. Regarding intake dated April 21, 2026. Through the investigation process, Licensing determined that the Licensee, by admission, failed to submit incident reports documenting multiple facility closures related to service delivery disruptions due to the Licensee's illness.

    Corrective action. Ensure a licenses notifies the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s required by Licensee. Licensee to submit a a corrective action plan to Licensing specific to the contravention.
