  Observation. Licensing observed surfacing around the outdoor play space did not meet DOLSOP requirements. Surfacing was tested in multiple locations around the slide, rope ladder and climbing wall (52" heights) and observed 1" surfacing with hard pan below. Licensee stated the climbing equipment will be offlimits to children until Licensing can come back out to test the surfacing.

  Corrective action. Ensure depth guidelines for loose fill material meets DOLSOP requirements.

  Observation. Licensing observed that the communication [minor incident log] book was not being kept current with regular entries.

• 3000 - SPS DOLSOP (Depth)

  Observation. Licensing tested protective surfacing depth (pea gravel) in multiple locations in the fall zone of the composite elevated play structure measuring fifty five (55) inches in fall height. Licensing observed there was zero to two inches of depth between the slide and the climbing ramp, with hard pan visible in the fall zone.

  Corrective action. Ensure depth guidelines for loose fill material is minimum six inches for equipment up to five feet in height.

  Observation. Licensing observed five children's files. Two files were missing immunization dates.

  Corrective action. Ensure current records must be kept for each child in care, including immunization status.

• A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child

  Observation. Licensing observed the following hazards in the outdoor play area; exposed nails, hard pan below platform of outdoor climber, two pieces of broken, sharp edged metal.

  Corrective action. Ensure children do not have access to any object/ substance that may be hazardous.

  Observation. Applicant stated the fire inspection has not yet been completed as they are still waiting to hear from the department.

  Corrective action. Ensure a licensee must have emergency exits and a fire drill system approved by a local assistant within the meaning of the Fire Services Act. Action/s Required by Licensing: Follow Up Inspection to be conducted.

• A 1.8 metre (6 feet) clearance around equipment is considered safest

  Observation. Licensing observed a wooden border around the climber separating the pea gravel and dirt is less than 1.8 metre (6 feet) from slide exit.

  Corrective action. Ensure a 1.8 metre (6 feet) clearance around equipment is considered safest. Action/s Required by Licensing: Follow Up Inspection to be conducted.
