  Observation. Licensing audited seven staff files and observed two files both missing copies of certificates or evidence of training.

  Corrective action. • A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

  Observation. Licensing audited 7 children's files and observed three files missing the child's Medical Services Plan number.

  Corrective action. Ensure for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status

  Observation. Licensing observed no outdoor play space checklist; Licensee states staff do check the playground for hazards but there is no log that is being maintained.
