    Observation. Licensing observed two pack n plays in the nap rooms with added mattresses to them. Manufacturers instructions state not to add any mattress to the pack n play. Licensee removed the mattresses while on site.

    Corrective action. Ensure that all indoor equipment accessible to children are free from hazards.
