  Observation. Licensing observed stagnant standing water in the sandbox, mud kitchen and tires, and Ice Salt that states "keep out of reach of children" accessible to children. Salt was removed from play space while on site.

  Corrective action. Ensure children do not have access to any object or substance that may be hazardous.

  Observation. Licensing observed daily outdoor checks had been done daily but had not been recorded consistently.

  Corrective action. Ensure record of daily, monthly and annual inspections.

  Observation. Licensing reviewed five of five staff files. One file was missing a valid criminal record check (CRC).

  Corrective action. Ensure not to employ a person without first obtaining: a valid CRC.
