• A licensee must keep a copy of the written statement, referred to in Section 19 of the Act, that the licensee delivers to persons who prepay part of the cost of services

  Observation. Licensing reviewed four child files and observed two files missing evidence that the Licensee had provided the parent with the Repayment policy; specifically two child files were missing a signed parent handbook/acknowledgment form that states the parent had read and agreed to the facility policies.

  Corrective action. Ensure to keep a copy of the written statement, referred to in Section 19 of the Act, that the licensee delivers to persons who prepay part of the cost of services.

• A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact

  Observation. Licensing reviewed four child files and observed one file missing record of the child's Medical Practitioner's name and telephone number and one file missing record of the child's Emergency Contact(s).

  Corrective action. Ensure keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
