  • For prevention of entrapment, spaces (i.e., holes/openings) must be smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). Use Head & Torso test probes

    Observation. Licensing observed spaces between the wooden slats on railing and between stairs of climber were between 3.5 inches and 9 inches. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. A plan for corrective action must be submitted to Licensing by June 15th, 2022, by 9 a.m. Full compliance will not be be achieved until the plan has been implemented and completed.

    Observation. Licensing used the entanglement device three times on each side of the slide. Test failed each of the six times. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. A plan for corrective action must be submitted to Licensing by June 15th, 2022, by 9 a.m. Full compliance will not be be achieved until the plan has been implemented and completed.

    Corrective action. Ensure equipment does not pose an entanglement risk.

    Observation. Licensing observed platform at top of slide and climbing wall was 63 inches high.

    Corrective action. Ensure equipment for preschool children must be no more than 1.5 meters (5 feet) in height.
