Observation. Licensing reviewed five of thirty-two children's records. Five children's records were missing signed copy of repayment agreement. Licensee stated that they are in the process of providing updated parent packages and signed repayment agreements.
Corrective action. Ensure that, for each child, there is a copy of the signed repayment agreement.
