Observation. Licensing observed that there was no sturdy surface for chnaging diaper in the Group Child Care (30 Months to School Age) program, Manager stated the change mat had broken and this is why they were using the change station located in the Group Child Care (Under 36 Months) program.
Corrective action. Ensure that a sturdy surface for changing diapers is provided for a Group Child Care (30 Months to School Age) program.

Observation. Licensing measured that a wooden table had been placed within 3' feet of a composite climber with slides, and a car tire was 3.4' away from the same composite climber structure.
Corrective action. Ensure a 6 feet clearance is considered safest.
