  Observation. Licensing measured wood chips surfacing under a dome climber measuring 5' in height to be 4" deep.

  Corrective action. Ensure a minimum of 6" of protective surfacing is provided for equipment measuring 5 ' in height.

  Observation. Licensing observed that there was no sturdy surface for chnaging diaper in the Group Child Care (30 Months to School Age) program, Manager stated the change mat had broken and this is why they were using the change station located in the Group Child Care (Under 36 Months) program.

  Corrective action. Ensure that a sturdy surface for changing diapers is provided for a Group Child Care (30 Months to School Age) program.

  Observation. Licensing measured that a wooden table had been placed within 3' feet of a composite climber with slides, and a car tire was 3.4' away from the same composite climber structure.

  Corrective action. Ensure a 6 feet clearance is considered safest.

  Observation. Licensing observed that a portable toilet was being used in the facility's main area designated for eating. Licenisng observed the potty was in use and full upon arrival at the facility. Item corrected at time of inspection.

  Corrective action. Ensure a program is established to instuct children in, and practice rules of health and hygiene.

  Observation. Licensing audited five staff files and observed one missing character reference.

  Observation. Licensing audited seven children's files and observed one missing emergency contact.
