Observation. Licensing audited seven children's files and one file did not contain emergency contacts.
Corrective action. Ensure records kept for each child contain emergency contacts.
3070 - CCLR 57(2)(d) - A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Licensing observed a staff member did not wash their hands and the child's hands after a diaper change. The staff member, after changing the diaper, continued their activities with the gloves on that were used during the diaper change and proceeded to handle children's toys and eating highchairs.
Corrective action. Ensure to establish a program to instruct children in and to practice rules of health and hygiene.

Observation. Licensing observed a log of all inspections, maintenance and repairs was not kept for the Group Child Care (School Age) program.

3070 - CCALA 7(1)(b)(i) - A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.
Observation. Licensing audited seven children's files and two files did not contain immunization status.
Corrective action. Ensure a records for each child are showing an immunization status.
3070 - CCLR 57(2)(a) - Without limiting subsection (1) a licensee must ensure that children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E, (except FCC/IHMACC/REC; child qualified if in age range or within one month (except GCC School Age)).
Observation. Licensing observed that a child enrolled in the Group Child Care (School Age) program was recorded in the attendance records for the Group Child Care (30 Months to School Age) program. The Manager stated that there was only one child registered in the Group Child Care (School Age) program and that child was currently attending the Group Child Care (30 Months to School Age) program so that the child would not be alone and was therefore recorded on that program's attendance sheet. There was no TPR approved for that child.
Corrective action. Ensure that children are divided into groups with each group having no more children than that permitted in Schedule E.

Observation. Regarding intake September 18, 2025. Through the investigation process, Licensing determined that behavioural guidance practices were not age appropriate as staff were frequently physically moving children to guide behaviours.
Corrective action. Ensure that the behavioural guidance is appropriate to the age and development of the child who is receiving the guidance. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On October 15, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: A staff meeting was held on October 15, 2025, to review policies and procedures related to behavioural guidance. The corrective action plan was reviewed and accepted by Licensing.
3030 - CCLR 51(1)(a) - A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Observation. Regarding intake September 18, 2025. Through the investigation process, Licensing determined that a staff restrained a child as emergency restraint due to challenging behaviours. The incident was not notified to the Medical Health Officer.

Observation. Regarding intake September 18, 2025. Through the investigation process, Licensing determined that a child was restrained by a staff member for behavioural guidance and it was not authorized by parents in care plan.

Observation. Licensing observed five portable slides placed directly on concrete. The slides measured between 0.53 cm and 0.63 cm in height and were used by both the Group Child Care (Under 36 Months) and Group Child Care (30 Months to School Age) programs. The woodchips were unevenly distributed, and there was no depth to form a sufficient surface to reduce risk of injury from falls.
Corrective action. Ensure all outdoor play equipment accesible to children are safe and free from hazards and in good repair.

Observation. Licensing observed a medical sheet signed by a physician authorizing the use of an epi-pen. The form was not signed by a parent and did not have complete information as required in a care plan. Staff stated that they did not think the child was allergic at this time but was not sure if an epi-pen was on site.
