  • A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

    Observation. Regarding intake May 23, 2025. Through the investigation process, Licensing determined staff had not reported to Licensing within 24 hours, that a child had been unaccounted for, for approximately 60 minutes.

    Corrective action. Ensure to notify the Medical Health Officer within 24 hours. after a child is involved in, or may have been involved in, a reportable incident described in Schedule H, while under the care or supervision of the Licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 13, 2025 and June 18, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: staff would receive education of Reportable Incidents and would be required to document any incidents in the classroom communication book. Managers to text or email daily the Licensee Contact(s) in order to confirm if an incident is Reportable or Non Reportable so Management can document if necessary. Licensee contacts would report to Licensing any reportable incident within 24 hours of the incident occurring. Licensee Contact(s) would review staff implementation of documenting and reporting to management any incident, by conducting bi-weekly visits to each facility/classroom and reviewing the classroom communication books; Licensee Contact(s) bi-weekly visits would be documented in the classroom communication books. The corrective action plan was reviewed and accepted by Licensing on June 24, 2025.

  • A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

    Observation. Regarding intake May 23, 2025. Through the investigation process, Licensing determined staff had not ensured the health, safety and dignity of a person in care, specifically that the terms and conditions of the facility licence had not been compliant when groups had combined and the maximum capacity identified on the facility licence had been exceeded.

    Corrective action. Ensure to operate the community care facility in a manner that will promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 13, 2025 and June 18, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: staff would receive education of a new attendance procedure and maximum group size for each classroom, to ensure all children are accounted for and maximum group sizes were not exceeded. Licensee Contact(s) would review staff implementation of the new attendance procedure by requiring all Managers to document daily attendance in each classroom communication book and all Managers would be required to text daily, the Licensee Contact(s), the number of children present in each classroom; all staff would be required to sign and date having reviewed and understood the new attendance procedure and maximum group sizes. The corrective action plan was reviewed and accepted by Licensing on June 24, 2025.
