  Observation. Licensing obsevered nine staff records, five staff records were missing an immunization status.

  Corrective action. Ensure for each person employed in a community care facility an immunization status is obtained.

  Observation. Licensing reviewed nine staff records, 1 staff record was missing a resume.

  Corrective action. Ensure for each person employed in a community care facility, a resume is obtained.

• A licensee must not employ a person in a community care facility as an educator or an assistant unless the person holds a valid certificate issued under Division 2 [Employee Qualifications], which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry

  Observation. Licnesing reviewed nine staff records, four staff records were missing a certificate validation.

  Corrective action. Ensure for each person employed in a community care facility as an educator or an assistant, holds a valid certificate under Division 2 and the Licensee has verified certification.

  Observation. Licensing reviewed ten children's records out of thirty-one; one record was missing a physician telephone number.

  Corrective action. Ensure for each child's record a physician telephone number is obtained.

• A licensee must not employ a person in a community care facility as an educator or an assistant unless the person holds a valid certificate issued under Division 2 [Employee Qualifications], which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry

  Observation. Licensing reviewed seven staff records; 2 staff records were missing certificate validation.

  Corrective action. Ensure for each staff record the ECE / ECEA certificate is validated.
