• A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure

  Observation. Licensing observed attendance record for the day of this inspection. Two children had left the facility to attend school however they had not been signed out on the attendance record. The attendance record showed five children in care but only three children were in care.

  Corrective action. Ensure an accurate record is kept of the time of arrival and departure for all children in care.

  Observation. Licensing reviewed four children's records, four children did not have a photo or digital image to identify them in an emergency.

  Observation. Licensing reviewed four children's records. Three children were missing MSP numbers and three children were missing immunization records.

  Corrective action. Ensure all children's records include immunization status and MSP number prior to enrollment.

  Corrective action. Ensure all children in care have time of arrival and departure and whether the child is absent on the attendance records.

  Corrective action. Ensure all children's records include the name and phone number of a doctor or walk in clinic.
