  Observation. Licensing observed that the fire extinguisher needed to be serviced on or before June 4, 2025.

  Corrective action. Ensure that fire extinguisher's are being serviced annually.

  Observation. Licensing observed that the volunteers file was missing a immunization declaration.

  Corrective action. Ensure that every employee, including volunteers, has immunization status/declaration on file.
