    Observation. Regarding intake August 14, 2025. Through the investigation process, Licensing determined that while in care, a child was involved in a reportable incident as per Schedule H, which was not reported to Licensing within 24 hours.

    Corrective action. Ensure that the reportable incidents, per Schedule H are reported to Licensing within the 24-hours. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On October 3, 2025, Licensee submitted a corrective action plan to Licensing.
