Observation. Licensing observed that four children's records did not have consent to release to someone other than the parent in writing from the parent.
Corrective action. Ensure that consent to release a child other than to the parent is in writing.
3070 - CCLR 57(3)(b) - A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.
Observation. Licensing observed three medical service numbers in children's records were missing.
Corrective action. Ensure that all required children's records are up to date.
