    Observation. Regarding intake dated February 19, 2026. Through the investigation process, Licensing determined parents were not immediately notified of children's injuries.

    Corrective action. Ensure that a parent or emergency contact is immediately notified if a child becomes ill or is injured. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 17, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the staff handbook would be updated to state that all head injuries, regardless of severity, required an immediate phone call to a parent and a mandatory staff meeting would be held focusing on immediate notification protocols for illness and injuries. The corrective action plan was reviewed and accepted by Licensing.

Some of the push pins were loose on the large paper, attached to the wall in the main play area ( in two rooms) that could easily fall or be pulled out.
