Observation. Regarding intake March 30, 2026. Through the investigation process, Licensing determined that a care plan for a person in care was not followed and was not updated to include strategies to guide behaviours such as physical guidance and use of sign language.
Corrective action. Ensure to operate the community care facility in a manner that will promote the health, safety & dignity of persons in care, specifically, following care plans. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On April 24, 2026, Manager submitted a corrective action plan to Licensing. The corrective action plan included: Care plans would be updated immediately and accurately to reflect the supports provided to children who required them. Observation. Regarding intake May 28, 2025. Through the investigation process, Licensing determined that a practicum student acted as a qualified employee when released a child to a parent and was directly supervised a child in the outdoor area.
Corrective action. Ensure children are adequately supervised by qualified employees in sufficient numbers to meet the needs of each child. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between June 25, 2025 and July 4, 2025, Manager, submitted a corrective action plan to Licensing. The corrective action plan included: A staff meeting was held on May 30, 2025 with all staff and members of the Board to review the incident occurred, policies, staff and practicum students responsibilities regarding supervision. Staff and practicum students were reminded that students were not included in ratio, left alone with children nor act as qualified staff. Additionally, staff reviewed and signed a diagram indicating the requirement of supervision by a minimum of two staff to always see all areas of the playground. The corrective action plan was reviewed and accepted by Licensing.

Observation. Regarding intake May 28, 2025. Through the investigation process, Licensing determined that a child was left in the outdoor area without supervision.
Corrective action. Ensure children are supervised at all times by an educator/assistant. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between June 25, 2025 and July 4, 2025, Manager, submitted a corrective action plan to Licensing. The corrective action plan included: a staff meeting was held on May 30, 2025 with staff and members of the Board to review the incident occurred, policies and staff responsibilities regarding supervision. Staff meetings would be held on a quarterly basis for professional development and to continue to review policies. Additionally, staff members reviewed and signed a diagram of the playground area indicating the requirement of supervision by a minimum of two staff to always see all areas of the playground. The corrective action plan was reviewed and accepted by Licensing.

Through the investigation process, Licensing determined that determined that the Licensee did not notify the medical health officer within 24 hours after the child was involved in a reportable incident described in Scheduled H while under the care of the licensee.
Corrective action. Ensure the Medical Health Officer is notified within 24 hours if child had been or may have been involved in a reportable incident described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. The corrective action plan included: All reportable incidents would be communicated to Licensing via phone or email within 24 hours. The corrective action plan was reviewed and accepted by Licensing.

Observation. Regarding intake May 6, 2025. Through the investigation process, Licensing determined that the supervision policy was not followed by staff when a child was left outdoors alone and when staff did not notice and intervene when two children where fighting over a toy as indicated in their policy.
Corrective action. Ensure that policies and procedures are implemented by employees. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Corrective action. Ensure children are supervised at all times by an educator/assistant. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On May 27, 2025, Manager, submitted a corrective action plan to Licensing. The corrective action plan included: Attendance sheets were created for both licenced groups to help staff to track children throughout the day. Staff would count children regularly and conduct a final sweep of every area before leaving to ensure supervision at all times. Staff members involved in the miscounting of children were not longer employed. The corrective action plan was reviewed and accepted by Licensing.

Observation. Regarding intake May 6, 2025. Through the investigation process, Licensing determined that practicum students were acting as qualified employees and the ratio of employees to children attending the facility was less than permitted in Schedule E.

3070 - CCLR 57(2)(d) - 3000 - SPS DOLSOP (Entanglement)
Observation. Licensing used the entanglement device and detected entanglements on both sides of the red slide. This represents an immediate risk to the health and safety of a child and must be corrected immediately. Manager stated that the slide would not be used until the entanglement was corrected and tested by Licensing.
Corrective action. Ensure equipment is free of entanglement hazards in areas of uncontrolled motion.
