• A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

  Observation. Regarding intake April 29, 2026. Through the investigation process, Licensing determined a reportable incident that occurred on March 24, 2026, was not reported to the Medical Health Officer within 24 hours.

  Corrective action. Ensure to notify the Medical Health Officer within 24 hours after a child is involved in or may have been involved in a reportable incident described in Schedule H while under the care or supervision of the Licensee. Action/s Required by Licensee: Licensee Contact to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 2, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: Staff would reinforce reportable incident procedures and staff would receive ongoing training on reporting, communication, and documentation. Tracking systems would be kept to ensure accurate monitoring and record keeping of incidents. The corrective action plan was reviewed and accepted by Licensing.

• A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure

  Observation. Licensing observed attendance records had the arrival time, however, in December 2025, five attendance records were missing the departure time.

  Corrective action. Ensure that a record is kept for each child showing the following information: daily attendance record, indicating for each day whether the child is absent or, if the child is present, the arrival and departure time.

  3070 - CCLR 57(2)(c)
• A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact

  Observation. Licensing observed eleven children's files. Two children's files were missing an emergency contact.

  Corrective action. Ensure that a records is kept for each child showing showing the following information: name and telephone number of a parent, medical practitioner and emergency contact.

• If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child’s parent or in the child’s record or care plan

  Observation. Licensing observed a medication was prescribed to a child by a medical practitioner and the medication was provided to the facility by a parent. There was no record what the Licensee had agreed to with the parent and there was no indication of the amount or times indicated by the child's parents in the child's record.

  Corrective action. Ensure that if a Licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the Licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record.

  3040 - CCLR 53(1)(a)
• A licensee must have in writing from a parent, and maintain at the community care facility, consent (b) to release a child to someone other than the parent.

  Observation. Licensing observed eleven children's files. Two children's files were missing written consent to release a child to someone other than a parent.

  Corrective action. Ensure that a a record is kept in writing from a parent, and maintained at the community care facility, consent to release a child to someone other than the parent.

• A licensee must keep, for each child, a record showing the following information:(i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency

  Observation. Licensing observed eleven children's files. Two children's files were missing a photo or digital image of the child.

  Corrective action. Ensure that a record is kept for each child showing the following information: a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.

• A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

  Observation. Licensing observed 11 children's files. One children's file was missing the medical insurance number and immunization status.

  Corrective action. Ensure that a record is kept for each child showing all of the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
