Observation. Regarding intake November 17, 2025. Through the investigation process, Licensing determined that children had access to substances that may be hazardous to their health and safety such as cleaners in the countertop where the diaper mats were placed.
Corrective action. Ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On December 10, 2025, Manager submitted a corrective action plan to Licensing. The corrective action plan included: All disinfectants and cleaning supplies would be stored in a cupboard with lock away from children's reach. The corrective action plan was reviewed and accepted by Licensing.

Observation. Licensing observed that plans had not been submitted to Licensing prior to moving walls to increase the size of room 3 and removing the approved diaper change area. On November 19, 2024, Licensing observed that additional structural change had occurred. Licensee removed a wall which increased the size of room 1 and eliminated the separate nap area assigned to room 3.
Corrective action. Ensure plans are submitted to the MHO before any structural changes are made to the facility.

Observation. Licensing reviewed six staff files. Two files did not contain a criminal record check. Staff stated that the Manager had applied for the CRCs.
Corrective action. Ensure a criminal record check is obtained for all employees.

Observation. Licensing observed staff change a child's diaper then use hand sanitizer. Staff did not wash hands before attending to the needs of another child.
Corrective action. Ensure appropriate handwashing practices are used as part of an established program to instruct children in and to practice the rules of health and hygiene.
