  Observation. Licensing audited four children's file, one was missing consent to release a child to someone other than a parent.

• A licensee must keep all criminal record check results and character references for the entire time that the subject of the criminal record check or character reference is ordinarily present on the premises.

Manager stated the maintenance worker was employed by the church and was on site on most days, and accessed a storage closet located in the downstairs licensed space.

  Corrective action. Ensure a Criminal Record Check is obtained for persons who are ordinarily present.

  Observation. Licensing observed that a log of maintenance and repairs was out of date, the last recorded date was June 1, 2023 for the portable and May 8, 2023 for the downstairs program.

  Observation. Licensing observed that the Corrective Action Plan to mitigate the risk of lead in water by conducting a two minute flush and recording it in a log was not being followed, the log had not been filled since May 8, 2023.
