Observation. Regarding intake date December 16, 2025. Through the investigation process, Licensing determined that children had access to hazardous objects in the classrooms due to exposed hardware and construction materials.
Corrective action. Ensure that children do not have access to any objects or substances that may be hazardous to their health and safety. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On January 9, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: on December 17, 2025, temporary physical barriers and protective covers had been installed to exposed hardware and construction materials. Permanent installation, re-adhesion and fixation of panels, baseboards and sink would be completed by February 5, 2026. The corrective action plan was reviewed and accepted by Licensing.
