Observation. Licensing observed that surfacing under equipment measuring 5' in height was 1" under the slide section of the equipment.
Corrective action. Ensure there is at least 6" of protective surfacing for equipment that's is 5 ' in height.

Observation. Licensing observed that a plan to mitigate the risk of lead in water was not being followed. Licensee stated the water was being flushed every morning but the log had not been kept as per the Corrective Action Plan.
