Observation. Licensing observed no record of two minute flush at the beginning of the day as stated in the corrective plan. Licensee stated the children did not drink from the bathroom sink, and she maintained a two minute flush in the morning, but had not been recording.

Observation. Licensing observed that the play room was now a personal gym and closed off to the rest of the usable space. Licensing also observed that the storage/garage area was now the play room. Licensing was not notified of this change.
Corrective action. Ensure Licensing is notified of any changes in information under Section 9 and Schedule B.
