    Observation. Regarding intake August 12, 2025. Through the investigation process, Licensing determined that the max group size as per Schedule E, which states the max group size is 12 was exceeded when the Group Child Care (Under 36 Months) programs combined and had a total of 22 children.

    Corrective action. Ensure the health, safety and dignity of person in care, specifically ensuring the maximum capacity is maintained. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: between September 19, 202 and November 7, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: Staff were to conduct head counts, and the manager would cross check to ensure maximum group size was not exceeded. The corrective action plan was reviewed and accepted by Licensing.

    Observation. Regarding intake August 12, 2025. Through the investigation process, Licensing determined that the staff to child ratios were not maintained when there was one staff (ECE) with thirteen children in the Group Child Care (30 Months to School Age) program and Educator lunch breaks were being covered by a less qualified Educator.

    Corrective action. Ensure staff to child ratio is maintained as per Schedule E. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: between September 19, 202 and November 7, 2025, Licensee submitted a corrective action plan to Licensing.     Observation. Regarding intake July 22, 2025. Through the investigation process, Licensing determined there was one staff ECE (replacing absent ITE) in the Group Child Care (Under 36 Months) program with six children, when an additional ECE was required in a group of 5-8 children. On November 28, 2025, Licensing observed at 8:22 a.m. Licensing observed two staff (ITE, ECEA) with five children in the Group Child Care (Under 36 Months) program. This did not meet the Schedule E, when ITE was required for 1 to 4 children and an ITE and ECE was required for 5-8 children.

    Corrective action. Ensure children attending are divided into groups, with each group having no more children than that permitted in per Schedule E. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: between September 19, 2025 and November 7, 2025, Licensee submitted a corrective action plan to Licensing.     Observation. Licensing observed the corrective action plan specific to the staff to child ratio was not being followed. There was one ECEA with one child when one ITE was required for one to four children. There was two staff (ITE, ECEA) with six children when an ITE, ECE was required for five to eight children.

    Observation. Regarding intake July 22, 2025. Through the investigation process, Licensing determined that on August 22, 2025, the staff to child ratios were not maintained when the Group Child Care (Under 36 Months) room had three staff (ECE, ECE, ECEA) with ten children. The Infant and Toddler Educator was scheduled off on August 22, 2025. This did not meet Schedule E when one Infant and Toddler Educator, one other Educator and one Assistant was required for 10 children in care. In addition, employee's lunch breaks were being covered by a lesser qualified Educator. On November 28, 2025, at 7:57 a.m. Licensing observed one staff (ECEA) in the Group Child Care (Under 36 Months) program with one child. The staff stated they had started their shift early in this room and the parent had completed the transfer of care in this room instead of the assigned one. The staff stated the ITE was coming at 8:00 a.m. from the designated Group Child Care (Under 36 Months) drop off room. The ECEA went to the designated room with the child. Licensing observed the staff break scheduled that indicated qualified staff were to replace staff lunch breaks.

    Corrective action. Ensure staff to child ratio is maintained as per Schedule E. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: between September 19, 2025 and November 7, 2025, Licensee submitted a corrective action plan to Licensing.
