  Observation. Mini Play Pen was being used by a child over one year of age. There was minimal space within the play pen as it was as long as the child. Additionally, one side of the play pen was collapsed, staff indicated that the child may have pushed this side down. Discussion had with staff and Manager about age appropriate sleep equipment and ensuring that the play pen is set up appropriately as per manufacturing instructions.

  Observation. Licensing could not confirm whether an emergency drill was conducted in the last year as there was no documentation to indicate it had been carried out. Manager stated that it had been carried out in August. Discussion had about ensuring that the drill is documented in order to confirm compliance.

  Observation. The following was noted: -Some areas where the paint has peeled from the wall, in particular in the smaller play room along the ledges by the duct taped areas. -Fence door and gate at entry are both broken. Staff was using a black hair elastic to lock the gate for the playground which is not considered an appropriate measure for securing the space.

  Corrective action. Please provide a written response to indicate how you will address the contraventions noted above on or before May 16, 2024

  Observation. There is a child in care since May 1, 2024 who does not have a file on site. Staff stated that they had just started at the facility and had not collected their information. There is another child who in care who does not regularly attend that does not have a file on site. Discussion had with staff about these children considered to be in care.

Observation (CORRECTED DURING INSPECTION): Immunization status was not provided within the sub staff file. Staff was able to produce the document at the time of inspection.

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. Licensing observed the following: - Chipped paint and chipped wood inside at the entrance. - Popcorn and dirty tissues on the ground in one of the play rooms. - A soccer net, in the outdoor grass area, that has ripped in a couple of areas. The Manager informed Licensing that it will be removed. - The grass in the outdoor play area is grown high. - A lot of clutter on the kitchen counter-top with no space.

  Corrective action. Ensure that the community care facility and furniture, equipment and fixtures within it are clean and in good repair while children are in care.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Licensing observed the following: - Four sharp nails sticking out in the bathroom where the toilet paper holder was. - An adult dirty glass cup, bowl and spoon sitting on one of the sleeping mats in the nap room. The Manager removed the items and placed them in the sink.

  Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.

• HYGIENE AND COMMUNICABLE DISEASE

  A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

  Observation. Licensing observed all the beds and bedding touching one another in the nap room. The Manager informed Licensing that there was a party yesterday. Licensing discussed the importance of bedding being separate to mitigate the spread of germs.

  Corrective action. Ensure that a program is established to instruct children in and to practise the rules of, health and hygiene.

  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Upon a random review of four children's records, Licensing observed one child's record missing their medical insurance number.

  Corrective action. Ensure that children's records are complete.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): Licensing observed the Manager completing the attendance sheet. Upon discussion, Licensing became aware that the Manager was completing the attendance record for the entire past week. Licensing discussed the importance of ensuring that attendance records are completed daily.

  Corrective action. Ensure that attendance records are accurate on a daily basis.

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. Upon a random review of four children's records, Licensing observed the following: - Two children missing a medical practitioners name and telephone number. - One child missing the telephone number for the emergency contact person.

  Corrective action. Ensure that children's records are complete.

  Observation. Licensing was unable to review the minor incident book, fire and emergency drills (years 2022 and 2023) because the Manager had taken them home. Licensing had a discussion to ensure that all facility records need to be in a single place within the community care facility.

  Corrective action. Ensure that all records referred to in sections 56 to 58 are kept in a single place at the community care facility

  In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.

  Observation. The Manager informed Licensing that there are two children in care that do not have records available at the facility.

  Corrective action. Please ensure children's records are available and kept in a single place in the community care facility.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  Observation. Licensing observed the following that are accessible to children in care; - a facial cleanser bottle on the bathroom counter. -hazardous gardening tools in an unlocked shed - a cleaning solution spray bottle in an unlocked bathroom cupboard. The Manager informed Licensing that the lock has broken. The Manager removed the cleaning spray bottle.

  Corrective action. Please ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Licensing observed the following:- - There is no longer any grass on half of the outdoor grass area. It is now muddy and not sufficient resilient surfacing. - The bleach and water spray bottle is not labelled. - several wooden drawers stacked on top of one another in the bottom cement outdoor area. - a plugged in vacuum cleaner sprawled on the nap room floor. - a medium sized photocopier sitting on the floor on three workout step stools in the nap room.

  Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are in care.

  Observation. Licensing observed that there was no evidence of an emergency drill being implemented. The Manager informed Licensing that an emergency drill was implemented but that they were unaware that it needed to be recorded. Licensing provided education that an emergency drill needs to be practiced at least once a year and needs to be recorded.

  Corrective action. Please ensure that current records are kept for all fire and emergency drills that have been implemented.

The Manager informed Licensing that the parents have not provided the child's picture yet. Licensing provided clarification that it is the responsibility of the Licensee to have a picture of all the children in care.
