  Daily inspections of the play space for garbage, broken glass, animal feces and other hazardous items have not been done. ; An annual audit is not performed on all play spaces, landscaping and equipment. ; Monthly detailed inspections of the play space have not been conducted, including a check of the landscaping and fencing and overall condition of equipment to ensure that they do not pose any hazards to children. ; Check lists that help to ensure checks and inspections are performed regularly and that proper maintenace and repair are completed are not available for staff.
