Observation. Licensing observed the following: - two children at separate times climbing on a chair in the outdoor play space positioned above concrete - a soiled fabric cover for the diaper change mat - two uncovered electrical outlets in the children's nap room - two unsecured blind cords potentially accessible to children posing a potential risk of entanglement - the latch is no longer in place restricting children's access to a family bedroom that has not been approved for children's access, containing hazards such as unsecured cords

Corrective action. During the inspection, the licensee removed the chair, changed the diaper mat cover, covered the electrial outlets and secured the blind cords. The licensee will secure the bedroom to ensure that children do not have access.
