- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed that the locking system of the metal gate located outdoors to enter the outdoor play area was broken (unable to close).
Corrective action. For the safety of children particularly when they are outdoors, ensure that the gate is repaired and be able to be locked.
11020 - CCLR s.13(2) - OR SUPERVISION
A licensee must ensure that children are adequately supervised by qualified employees in sufficient numbers to meet the needs of each child.
Observation. During the inspection, the only individual that was present on-site with the children, identified as a volunteer to Licensing.
Corrective action. Volunteers must not be left alone to care for children, and requires the supervision of a staff member at all times.
14010 - CCLR s.34(1) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Licensing observed that the last recorded fire drill was back on August 16, 2024. There were no records of recent emergency drills as well completed with children. The individual present with childen said that these were the only records of fire and emergency drills available on-site. This is a repeat contravention on routine inspection report #MZIN-D5ZMUP on 07/Jun/2024.
Corrective action. Fire drills must be completed with children and recorded monthly. Emergency drills must be completed with children and recorded yearly.
13050 - CCLR s.56(1)(c) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. The fire extinguisher was due to be serviced on May 02, 2025.
Corrective action. Ensure that the fire extinguisher is serviced for safety and in case of an emergency.
13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.
Observation. The consent for medical attention for one child was not available on-site.
Corrective action. All requirements of CCLR s. 57 must be obtained by children's first day of care, and ensure that the file is complete.
19190 - CCLR s.57(3)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.
Observation. One child was missing the date of enrolment on file. This is a repeat contravention on routine inspection #MZIN-D5ZMUP on 07/Jun/2024 & #CGEE-CNGT67 on 26/Jan/2023
Corrective action. Children's start date must be documented.
19080 - CCLR s.57(2)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. The individual that was alone with children today said that there is no system to record absent children. Licensing looked at previous attendance records, and observed that there are no records of absent children (including the attendance sheet during the inspection). This item is a repeat contravention on routine inspection report #MZIN-D5ZMUP on 07/Jun/2024.
Corrective action. Aside from children's daily time of arrival and time of departure, absent children on a daily basis must also be recorded.
19090 - CCLR s.57(2)(c) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Two children's doctor / clinic name and phone number were not available on file, and one child had the parents listed as the emergency contact. This is a repeat contravention on routine inspection report #MZIN-D5ZMUP on 07/Jun/2024 & #CGEE-CBRTS2 on 18/Feb/2022.
Corrective action. Ensure that for each child the doctor / clinic name and phone number is available on file, and the emergency contact should be different from the child's parents.
19100 - CCLR s.57(2)(d) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation. Two children's photograph / digital image were not available on-site. This is a repeat contravention on routine inspection report #MZIN-D5ZMUP on 07/Jun/2024.
Corrective action. All requirements of CCLR s. 57 must be obtained by children's first day of care, and ensure that the file for each children is complete.
19160 - CCLR s.57(2)(i) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. One child was missing the medical service plan personal health number on file, and three children were missing their immunization status record on file. This is a repeat contravention on routine inspection #MZIN-D5ZMUP on 07/Jun/2024 & #CGEE-CBRTS2 on 18/Feb/2022.
Corrective action. All requirements of CCLR s. 57 must be obtained by children's first day of care.
19070 - CCLR s.57(2)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.
Observation. The immunization record status were not obtained for two employees.
Corrective action. Ensure that staff files are complete, and all required documents are available on-site.
12090 - CCLR s.19(1)(f) - STAFFING
A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)
Observation. Three validation records were observed to be missing on file. This is a repeat contravention on routine inspection report #MZIN-D5ZMUP on 07/Jun/2024.
Corrective action. Certificates must be validated via the online system maintained by the Early Childhood Educator Registry.
12140 - CCLR s.19(4)(a) - STAFFING
A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.
Observation. Licensing observed that one staff did not have a first aid certificate available on file. The individual present at the facility today said that this staff is a regular employee at the facility. This item is a repeat contravention on routine inspection report #CGEE-CBRTS2 on 18/Feb/2022.
Corrective action. Ensure that this staff has a valid and up-to-date first aid certificate available on-site.
12210 - CCLR s.23(1)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.
Observation. It was observed that three character references were missing on file.
Corrective action. Ensure that all files are complete with required documents / records.
12050 - CCLR s.19(1)(b) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. The criminal record check (CRC) of the individual present on-site today was from a post-secondary institution. In addition, it was observed that two other staff's CRC were not available on-site.
Corrective action. All CRC's must be site specific, and CRCs must be available on-site.
12040 - CCLR s.19(1)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.
Observation. The resumes (record of work history) of three individuals were not available on-site.
Corrective action. Prior to start working at the facility with children, ensure that all files are complete and available on-site.
12060 - CCLR s.19(1)(c)

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: - The sharp tip of nails/screws were exposed and some were rusted (near the seats). These nails/screws were attached to the outdoor wooden picnic table with seating connected to it, and accessible to children. This furniture was located at the outdoor play area of the facility. - Sleeping mats with the bedsheets on were stacked together and touching (This item was corrected during the inspection).
Corrective action. The facility manager communicated to Licensing during the inspection that she will get rid of the outdoor furniture identified today. Ensure a safe environment is always provided to children while in care. A staff placed each of the children's mats around the nap room. Ensure that the bedsheets are not touching when stored and not in use.
11010 - CCLR s.13(1) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene. Observation (CORRECTED DURING INSPECTION): Licensing observed that there was no soap available in the bathroom for children to use when washing their hands.
Corrective action. A staff moved the soap dispenser container which had a pump, from the kitchen to the bathroom. The facility was informed by Licensing to have two completely separate soaps for the kitchen and for the bathroom.
15030 - CCLR s.46(1) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. The fire drill log for the year 2024 was not available on-site. The manager said that the monthly fire drills have been completed, though the records were removed on-site.
Corrective action. Ensure that this document is available on-site going forward.
13050 - CCLR s.56(1)(c) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation. One child did not have a photo on file.
Corrective action. Ensure that the child has a complete file.

In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58. Observation (CORRECTED DURING INSPECTION): Licensing observed the following: - Staff files were not available on-site - The manager's CRC was not available on-site (This is a repeat contravention - Report # CGEE-CBRTS2 - 18/Feb/2022)
Corrective action. These items were corrected by the facility manager during the inspection, and the documents/files are now on-site.
19320 - CCLR s.59(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. Licensing observed the following: - One child's immunization record was not included on file - One child's gender was not noted on file - One child's PHN was not included on file (This is a repeat contravention - Report #CGEE-CBRTS2 - 18/Feb/2022)
Corrective action. Ensure that children's files are complete prior to them starting in the facility.

Observation. Licensing observed two children's date of enrolment were not included on file.
Corrective action. Ensure that children's files are complete.
19080 - CCLR s.57(2)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): One child's time of arrival was not documented today on the attendance sheet.
Corrective action. Ensure to document children's time of arrival, time of departure and indicate if they are absent.

Observation. Licensing observed the following: - There was only a phone number written for the emeregency contact for one child, and this phone number was the same one as the parent's contact number. This was corrected by the facility manager during the inspection. There was also another child missing their emergency contact information on file. - Two children lacked a medical practitioner / clinic name and contact information on file. (This is a repeat contravention - Report # CGEE-CBRTS2 - 18/Feb/2022).
Corrective action. Ensure that children's files are complete.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation) Observation (CORRECTED DURING INSPECTION): The staff present today with the children did not have a validation record on file.
Corrective action. This was corrected by the facility manager during the inspection.

Observation. The lone employee's ECE certificate expired on 16/Nov/2022, which the manager explained was recently applied for.
Corrective action. The minimum employee to children ratio is 1 ECE : ≤ 8 children. CCFL & the manager discussed ways to come into compliance.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Two 58" tall storage units weren't anchored to the wall.
Corrective action. Furniture > 4' must be secured to the wall to prevent tipping over onto a child.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): 2 non-childproofed kitchen drawers held several knives.
Corrective action. The manager moved the knives to a high cabinet that's inaccessibible to children.
11200 - CCLR s.17 - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation. The manager isn't part of ratio but visits the facility frequently. She stated that her CRC is @ her other facility. FHA confirmed in the CCFL database that the manager holds a valid CRC.
Corrective action. A copy of the manager's CRC is required onsite.
19320 - CCLR s.59(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. CCFL reviewed all children's files & found that a child lacked his PHN.
Corrective action. All requirements of CCLR s.57 must be obtained by children's 1st day of care. If a child lacks a BC PHN due to being new to BC, this must be indicated on their file, & updated once they have a BC PHN.
19070 - CCLR s.57(2)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. CCFL reviewed all children's files & found that a child lacked his medical practitioner's contact information.
Corrective action. All requirements of CCLR s.57 must be obtained by children's 1st day of care. If a child lacks a Dr or walk-in clinic due to being new to BC, this must be indicated on their file, & updated once they have a Dr or walk-in clinic.
19100 - CCLR s.57(2)(d) - STAFFING
A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.
Observation. The employee's 1st aid/CPR certificate expired on 13/Oct/2021.
