A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation. Upon review of 8 children files, one file was missing a photo of the child.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: - indoor: the nap room had a baseboard heater that was duct taped in the corners, with the bottom section exposed and being hazardous if children play with it The manager said that it doesn't work and will talk to the church about getting it covered. - outdoor: The black tarp underlay is exposed beneath the woodchips and poses a tripping hazard for the children The manager said that recently someone came out to discuss with the licensee about outdoor area plans so she will follow up.
Corrective action. Ensure that the facility is clean and in good repair at all times when children are in attendance.
11020 - CCLR s.13(2) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,
Observation. Licensing observed that the children's bedding is stacked on top of each other which is a hygenic concern. The manager said that she will have the bedding separated with bins.
Corrective action. Please ensure that the children's bedding is stored separately while not in use and are not touching each other.
15050 - CCALA s.7(1)(b)(i) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed the fire extinguisher was not serviced annually, it expired earlier this month. The manager said the fire department came to the facility but the tag may not have been changed.
Corrective action. Ensure that all steps areas of the emergency plan are ready to go in case of an emergency, including the fire extinguisher being serviced annually
13020 - CCLR s.22(1)(b) - STAFFING
A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation) Observation (CORRECTED DURING INSPECTION): Licensing reviewed 3 staff files; 2 staff who hold a certificate were not verified through the Early Childhood Educator Registry.
Corrective action. Ensure that all staff's certificates are validated through the Early Childhood Eductor Registry upon hiring.

Observation. Immunization status was not obtained for one volunteer.
Corrective action. Licensee contact explained they will obtain immunization status.

Observation. The following was observed inside the facility: -tables in the 3-5 room are showing signs of wear and tear -the transition piece between the carpet and floor in the Preschool room is showing signs of wear and tear Outside: -the black tarp underlay is showing throughout some parts on the wood chips
Corrective action. The Director has explained that the tables will be monitored until they are replaced, the transition piece will be replaced and the wood chips will be replenished to cover the underlay..

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed that the two wooden table, in the Group Child Care( 30 mnths to SA), are still broken at the corners. The Manager informed Licensing that new tables have been ordered but not yet arrived.
Corrective action. Ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in care.
11020 - CCLR s.13(2) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E. Observation (CORRECTED DURING INSPECTION): Shortly after arriving, Licensing observed a 9th child arrive with only one staff and after 10-12 minutes a 10th child arrived, in the Group Child Care (30 mnths to SA) program. The second staff arrived a couple of minutes after the 10th child arrived. The staff to child ratio for this care program is 1:8. This is a repeat contravention. Licensing provided education on ensuring that staff to child ratio is maintained at all times. Discussed options to ensure that ratio is maintained.
Corrective action. Ensure that the ratio of employees to children attending the community care facility is no less than that permitted in Schedule E.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed a microwave on a shelf with the cord hanging, in the Preschool and Group Child Care (school age) classroom. The microwave and wire for it are accessible to children. Potential for a child to pull on its cord and have it fall on them and injure them.
Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; -In the outdoor playground the black underlay tarp is exposed in many areas. - Two planters completely broken and one wooden part of the planter is on the ground and the dirt has spilt everywhere in the wood chip area. - One planter half broken. - Six sleeping mats ripped and the foam underlay is exposed. -Two wooden tables, in the Group Child Care (30 mnths to SA) classroom, are broken at two corners.
Corrective action. Ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in care.
11020 - CCLR s.13(2) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E. Observation (CORRECTED DURING INSPECTION): Upon arrival Licensing observed nine children in care with one staff for 10 minutes before the secondary staff arrived. Licensing discussed staffing schedule based on the time of arrival of children.
Corrective action. Ensure that the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.
14030 - CCLR s.34(2)(b) - RECORDS AND REPORTING
A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.
Observation. Upon discussion with the Manager Licensing observed that there is a child who was recently diagnosed by a medical practitioner. However, upon review of the child's record the child's diagnoses was not recorded anywhere. Licensing discussed the importance of ensuring that a child's diagnoses is recorded in their records.
Corrective action. Ensure that for each child requiring extra support, a current care plan showing the diagnoses is kept.
19210 - CCLR s.58(1)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.
Observation. Upon a random review of four children's records, Licensing observed one child's record missing the date of enrolment.
Corrective action. Ensure that children's records are complete.
19080 - CCLR s.57(2)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Upon a random review of four children's records, Licensing observed that one child's record was missing the name and phone number of a medical practitioner.
Corrective action. Ensure that children's records are complete.

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.
Observation. Upon a review of two children's care plans, that have now been created, Licensing observed that one child's care plan identifies the child having a severe allergy and an epi-pen is in place. The care plan does not include the symptoms nor the courses of action required in the event of an allergic reaction. Licensing provided education on the importance of knowing the symptoms to look for and what to do,step by step, in an event the child requires extra support.
Corrective action. Please ensure that for each child requiring extra support, a current care plan is in place showing the courses of action recommended by health care professionals o address the needs of the child requiring extra support.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - two garbage bin container covers, in which all the earthquake supplies are stored in, are cracked and broken. - 13 of the sleeping mats are ripped exposing the underlay. Licensing provided education how the mats cannot be disinfected and sanitized effectively. - many areas in the outdoor play area where the black underlay tarp is exposed. Tripping hazard.
Corrective action. Please ensure that the furniture, equipment and fixtures within the community care facility are clean and in good repair.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that any heated water accessible to children is heated to no more than 49° Celsius.
Observation. Licensing measured the water in the bathroom at 54.2 degrees Celsius. Licensing provided education that heated water accessible to children should not be more than 49 degrees Celsius.
Corrective action. Please ensure that heated water accessible to children is heated to no more than 49 degrees Celsius.
11230 - CCLR s.47 - LICENSING
A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.
Observation. Licensing observed that there is an early childhood educator assistant staff that works every morning in the Group Child Care (30 months to school age) program but does not have their certificate posted anywhere within the community care facility.
Corrective action. Please ensure that all staff certificates or a copy of their certificate are posted in a prominent place within the community care facility.
10070 - CCLR s.11(4) - POLICIES AND PROCEDURES
A licensee must (a) ensure that a community care facility has (i) emergency exits that meet the requirements of the BC Building Code, and (ii) a fire drill system that is approved as definedby section 31 of the Fire Services Act.
Observation. Licensing observed that the fire extinguisher was supposed to be serviced by November 10, 2021, however, has not been serviced.
Corrective action. Please ensure that fire extinguishers are serviced annually, before or by the required date.
13010 - CCLR s.22(1)(a) - RECORDS AND REPORTING
The licensee must (a) develop the care plan in consultation with a parent of the child requiring extra support and any person requested by the parent.
Observation. Licensing observed that there are two children who have severe allergies and epi-pens in the event of an allergic reaction. However, there are no care plans in place for both of the children. Licensing provided education on the importance of having care plans in place; describing the symptoms and courses of actions in the event of an allergic reaction. Additionally, Licensing provided education on how often care plans need to be reviewed.
Corrective action. Please ensure that care plans are in place for children requiring extra support and are developed in consultation with a parent of the child requiring extra support.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Upon a review of six staff records, Licensing observed the following; - one staff missing their criminal record check (CRC). The Licensee contact informed Licensing that it has been applied for but not yet received. - one staff's CRC is not site specific.
Corrective action. Please ensure that staff records are complete.

- Despite this section, during the half hour before a care program begins and the half hour after a care program ends, a licensee may permit children who would otherwise be grouped in different care programs, and who are arriving at or leaving the community care facility, to be grouped together and supervised other than as required under Schedule E if (a) the care program is described as Multi-Age Child Care, Group Child Care (Under 36 Months), Group Child Care (30 Months to School Age) or Group Child Care (School Age).
Observation. Licensing observed that the Group Child Care (30 months to School Age) and Group Child Care (School Age) are mixed for more than half an hour.

Observation. One staff does not have a valid cleared criminal record check on file.
Corrective action. Any staff that work with the children, including staff that only provide transportation to school and back must have a cleared criminal record check on file.
12040 - CCLR s.19(1)(a) - STAFFING
Despite the employee qualifications required by Column 4 of the table in section 1 of Schedule E [group sizes and employee to children ratios], a licensee must replace an absent employee described in Column 1 of the following table who is absent (b) for 30 consecutive days or more with a person described in Column 3 opposite that employee and the row in Column 2 labelled as "≥ 30 days".
Observation. The facility has not had a Manager for longer than 30 days.
Corrective action. A Manager must be replaced before 30 days has passed. An application for amendment and Licensee Declaration form must be submitted as part of this process.
12370 - CCLR s.37(3)(b) - STAFFING
A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.
Observation. Two staff who are each alone with the children do not have valid first aid.
Corrective action. Children must have access to someone with valid first aid at all times, including in the vehicle for transportation.

Corrective action. Please ensure there is sufficient ground covering to prevent accidents.
