Observation. Staff did not immediately notify a parent when their child was injured.

19010 - CCLR s.55(1)(a)(1.1)

- Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.
Observation. An ECE IT and ECEA were supervising a group of 8 children less than 30 months old, where an ECE IT and ECE were required.
Corrective action. The Licensee will be applying for an exemption to bring them into compliance with staff to child ratio requirements.
14030 - CCLR s.34(2)(b) - A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation. A child was left in a bathroom for at least 5 minutes without supervision.
Corrective action. The Licensee will be notifying Licensing on how they will ensure the continuous supervision of children.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair.
Observation. - A climbing toy in the outdoor play space does not have the required clearance to ensure a safe fall zone. - The turf in the outdoor play space is curling on the edges creating a tripping hazard.
Corrective action. The Manager will move or remove the climbing toy and secure the edges of the turf.
11190 - CCLR s.16(4)(b) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. - Licensing observed that through out the facility walls and high touch surfaces are dirty. - No outdoor play space checklist used prior to April 23, 2025.
Corrective action. Manager will create daily and weekly cleaning checklists for each program. Manager will create daily/ monthly/ and annual outdoor play space checklists to ensure safety of play area and equipment.
11020 - CCLR s.13(2) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. - Licensing observed that the diaper changing pads are not being sanitized between each change. - The diaper changing pad used in the infant room is ripped and is unable to be sanitized properly.
Corrective action. Manager has said she will ensure each diapering area has a sanitizing solution available. Manager will purchase a new diapering pad for the infant program.
15030 - CCLR s.46(1) - NUTRITION AND FOOD SERVICES
A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,
Observation. - Licensing observed children's lunches being stored on shelves without a system to ensure the temperature of the food meets Food Safe Standards. - Licensing observed that each program is not using appropriate dish washing methods to clean the dishes that children are using to eat.
Corrective action. Magaer will ensure the children's lunches have ice packs' Manager will ensure each program has bins for rinsing and dry racks for drying dishes.
17130 - CCALA s.7(1)(b)(i) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed a smoke detector in the infant room beeping indicating a potential malfunction.
Corrective action. Manager said she will call for a technician to come and repair the detector.

Observation. During a walk through of the Group Child Care (30 mos to SA) washroom, the hot water temperature was tested today and the temperature was 53.9 degrees Celsius
Corrective action. The manager states the temperature will be adjusted to ensure it does not exceed 49 degrees Celsius

Observation. Portable Air Conditioner hose is stretched from the child care space across the front emergency exit door, making it difficult to access the exit safely
Corrective action. Please ensure emergency exits access is not hampered or blocked in any way

Observation. Refridgerator in Infant room does not contain a thermometer, therefore there is no evidence to support that the optimal food storage temperature is being maintained
Corrective action. Please ensure refridgerator temperature is reading between 0 - 4 degrees Celcius when storing infant food supplies

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. During the inspection the following was observed: -the fridge in one of the two programs was measuring at 8 degrees Celsius, where the children's lunches/snacks are stored -A large dome shaped climber was present in the outdoor playspace and had less than 1.82m of space around the edge of climber making the fall zone restricted and greatly increasing the risk of injury to children. The climber was reported by staff to be used by the Infant/Toddler program sometimes -In the infant/toddler outdoor play space, there were several strips of astro-turf (grass carpet) with approximately 50% of the edges of the strips raised, exposing the wood chips below and creates choking and tripping hazards for infants and toddlers
Corrective action. -Ensure all fridge temperatures are kept between 0-4 degrees Celsius to keep children's foods at safe temperatures -The licensee is responsible to ensure there is a minimum of 1.82meters around the edges away from the climbing structure to any and all hard surfaces (Such as fences, cement, the step down from the upper playspace to the lower, etc), as well as under the climber. Ensure there is an adequate fall zone to reduce risk of injury to children in care. -The climber is not appropriate for use by the I/T program as it measures taller than 0.9m, is required to no longer be used for the Infant/toddler program -Ensure all of the astro-turf (grass carpet) are not lifting at the edges to reduce the risk of falls and exposure to wood chips for infants and toddlers
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. -In the bathroom of the I/T program, there were a mop/bucket and vacuum in reach of children. -In the outdoor play space there were several pallets with exposed rusty nails and sharp, jagged edges, in reach of children.
Corrective action. Ensure the children in care do not have access to items which could be hazardous or dangerous to their health and well-being.
11200 - CCLR s.17 - LICENSING
A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Observation. The manager's name was not identified in any display available within the facility.
Corrective action. Ensure the manager's name is displayed in a prominent place in the facility.
10050 - CCLR s.11(1) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.
Observation. One child in care had a medication to be administered, but there no evidence of permission or instructions to administer from the parents within the facility.
Corrective action. Ensure all medications to be administered have permission and instructions by the parents provided to the facility.
16010 - CCLR s.53(1)(a) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. There was no evidence of fire drills have been completed in January and February 2022 in one of the programs. The manager stated all fire drills had been completed, but not recorded.
Corrective action. Ensure there is evidence of a monthly fire drill completed for all programs at the facility.
13050 - CCLR s.56(1)(c) - RECORDS AND REPORTING
A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.
Observation. Six children's files were reviewed, and in one of the six files, there was no evidence of a care plan as related the child's need for extra support. In discussion with staff, it was discussed there were two additional children who attend the facility, and who did not have care plans available onsite, as related to the children's needs for extra support.
Corrective action. Ensure all children in care who require extra supports have the appropriate care plans available to guide the staff in how to care for those children.
19210 - CCLR s.58(1)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Six chldren's files were reviewed and in one of the six files, there was no evidence of an emergency contact available.
Corrective action. Ensure all children's files have evidence of at least one emergency contact (who is not the parent).
19100 - CCLR s.57(2)(d) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Three staff files were reviewed during this inspection. In one of the three staff files, there was no evidence of a criminal record check. Staff informed licensing that the criminal record check form "just needed to be printed", but did not have access to have it printed during the inspection. The two other staff criminal record checks were not site specific, but were addressed to the previous owner/facility.
Corrective action. Ensure all staff employed have a valid criminal record check available within their staff files, which is addressed to the current owner and facility.
12040 - CCLR s.19(1)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.
Observation. Three staff files were reviewed and two of the files did not have evidence of the staff's immunization status available.
Corrective action. Ensure all staff have provided evidence of their immunization status.
