Observation. Observation. Licensing reviewed attendance records for April 2024 until the date of inspection and observed in room three, operating as either Group Child Care (30 Month to School Age) or Preschool (30 Month to School), 38 times that the daily departure time had not been recorded.
Corrective action. Ensure a Licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. Licensing observed no early childhood educator available for Group Child Care (30 Month to school Age) and no early childhood educator available for Preschool (30 Month to School Age); Licensing observed an expired staffing exemption and no staffing exemption application was on file for either classroom.
Corrective action. Ensure that the ratio of employees to children attending the community care facility is no less than permitted in Schedule E.
3030 - CCLR 34(2)(b) - A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.
Observation. Licensing reviewed 7 child files and observed four files missing record of immunizations and one file missing a medical services plan number.
Corrective action. Ensure to keep a record showing the following information: Name, sex, date of birth, medical insurance plan number and immunization status for each child in care.
3070 - CCLR 57(2)(a) - A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care
Observation. Licensing observed no two minute flush chart being maintained that indicated that the corrective action plan accepted by Licensing, was being implemented to conduct a two minute water flush daily, in order to mitigate the risk of the facility water testing higher than the allowable maximum concentrate of lead in water. Licensing observed the Manager state that the facility only utilizes bottled water and that a new corrective action plan for the use of bottled water only will be submitted to Licensing. Licensing also observed multiple jugs of bottled water and a water dispenser machine.
Corrective action. Ensure to operate the community care facility in a manner that will promote the health, safety and dignity of persons in care.
