A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following was observed: Indoors: - walls are visible dirty - chipped paint - drywall exposed - holes in the wall - kitchen cabinet broken - table and chairs are visibly dirty Outdoors: - garbage through outdoor space - broken toys - outdoor toys visibly dirty
Corrective action. Licensee will create a corrective plan.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Plastic gloves accessible in the washroom.

11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Nine child files reviewed, three did not have emergency contact information.
Corrective action. Licensee will obtain required information

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. Nine child files reviewed, one did not have MSP number.
Corrective action. Licensee will obtain required information.
19070 - CCLR s.57(2)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Four staff files reviewed, one did not have site specific CRC.
Corrective action. Licensee will obtain correct documentation.

- MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.
Observation. Licensing observed Polysporin in the first aid kit. The Manager said the Polysporin is administered when required for the children, no parent permission has been provided to administer this product.
Corrective action. Ensure parent permission is provided to administer products with a DIN or NPN number.
16010 - CCLR s.53(1)(a) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.
Observation. Emergency medication required for a child was not readily accessible to employees.
Corrective action. Ensure emergency medication is kept on site.
16020 - CCLR s.53(1)(b) - POLICIES AND PROCEDURES
If a person prepays part of the cost of services provided by a class of community care facilities designated by the Lieutenant Governor in Council, the licensee must, at the time of the prepayment, deliver to the person a written statement setting out the terms and conditions on which a refund of all or any of the prepayment will be made.
Observation. Licensing observed no prepayment agreement.
Corrective action. Ensure there is a prepayment agreement created and provided to all parents.
13080 - CCALA s.19 - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed the following: - Insufficient emergency supplies for all children and staff - Expired emergency supplies - Insufficient first aid supplies in all kits
Corrective action. Ensure all emergency supplies are replenished.
13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (c) produce records, on demand, to the medical health officer.
Observation. The minor incident log book was unavailable for review. The Manager said the Licensee contact took it off site for review.
Corrective action. Ensure all records are available on site.
19340 - CCLR s.59(c) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.
Observation. A selection of staff files were reviewed. Out of the 3 reviewed, all 3 files did not contain verified references.
Corrective action. Ensure staff files are complete.
12050 - CCLR s.19(1)(b) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.
Observation. A selection of staff files were reviewed, out of the 3 reviewed, 2 files did not contain immunization status.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees. Observation (CORRECTED DURING INSPECTION): There was expired medication in the fridge and it was also accessible to the children.
Corrective action. The Manager removed the medication and will have a discussion with the parent.
11010 - CCLR s.13(1) - RECORDS AND REPORTING
The licensee must (a) develop the care plan in consultation with a parent of the child requiring extra support and any person requested by the parent.
Observation. There was no care plan on file for a child that requires medication in the event of an emergency.
Corrective action. A corrective action plan will be submitted.
19250 - CCLR s.58(2)(a) - STAFFING
A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.
Observation. One staff did not have evidence of a valid first aid certificate on file. This staff cannot work alone with children if they do not meet this requirement.
Corrective action. A corrective action plan will be submitted.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)
Observation. One staff file was missing certificate validation.
Corrective action. A corrective action plan will be submitted.

Corrective action. Please ensure facility is safe at all times.
