Observation. One toilet in group child care (30 months to school age) not in use due to plumbing issues.
Corrective action. Ensure all toilets are in good repair.

A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair. Observation (CORRECTED DURING INSPECTION): Two bikes in outdoor play area missing pedals. REPEAT CONTRAVENTION
Corrective action. Manager removed the bicycles from play area.
11190 - CCLR s.16(4)(b) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. No evidence of care plans being reviewed annually with the parent or any person requested to by the parent.

Observation. Pedals missing from bikes in both outdoor play areas.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Licensing reviewed nine child files. Two files did not contain a medical practictioner. One file did not contain emergency contact.
Corrective action. Complete required information.
19100 - CCLR s.57(2)(d) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Licensing reviewed five staff files. One did not contain a current criminal record check.
Corrective action. Obtain current records.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the following that are accessible to children; - a bucket with several diaper rash creams on the counter, in one of the group child care (under 36 months) bathroom, where the children's diapers are change. - Lysol wipe container on a counter in the group child care (30 months to SA) program. - a staff's purse stored in an unlocked drawer, in the group child care (30 months to SA) program, that contains items that have medical ingredients in it.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - Several mats in the group child care (30 months to SA) nap room that are ripped - A leg broken on a wooden water/sand table in one of the rooms - Diaper change mat ripped in the group child care (under 36 months) bathroom - several balls in the group child care (under 36 months) playground that are deflated. - Cupboard door is broken - Several areas where the paint has chipped.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. There is a small climber and wooden slide in the group child care (under 36 months) however, there is no resilient surfacing around it ensure that children don't get hurt if they fall. Discussed fall zones.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.
11010 - CCLR s.13(1) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed that there was no food and water in the on wheels emergency container. Discussed ensuring that there is sufficient food and water in the bag for all the children and staff for 72 hours.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.
13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (c) produce records, on demand, to the medical health officer.
Observation. Licensing was unable to review two children's records as the Licensee contact was unable to find them after attempting to locate them for some time.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.
19340 - CCLR s.59(c) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.
Observation. Upon a random review of three staff records, Licensing observed that immunization status was not obtained for two staff.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

A licensee must ensure that each employee (c) practices implementing the emergency plan at least once each year.
Observation. Upon discussion with staff Licensing determined that an emergency drill was not implemented for 2023 or 2024. Discussed the importance of ensuring that emergency drills are practiced.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

Observation. A staff member was observed yelling at a child while holding their arm "you have to listen to me" and "you have to pick it up" while on a walk.
Corrective action. Ensure that behavioural guidance is age and developmentally appropriate at all times.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following was observed: - the cabinets are chipped and the material is coming off in some areas; this is throughout the facility in the washrooms and main play areas - the paint is chipped on the walls and the drywall is exposed in areas thoroughout the facility; children are picking at the exposed drywall - 2 diaper change mats are torn and cannot be sanitized effectively - there is a small area of carpet in the toddler room that is torn; children can pull at the exposed edges that are frayed - one table in the 3-5 room is chipped on the edges and cannot be sanitized effectively - some tiles on the ground in the outdoor play space in the infant/toddler play area have large gaps between the tiles which pose as a tripping hazard
Corrective action. Ensure the facility is in good repair at all times
