- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed a new rubber surfacing that has been installed inside the community care facility however, the rubber is not flat and uneven in some areas. Potential for a child to trip, and injure themselves.
Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - paint chipped on a wooden bench and table in the outdoor play area - paint chipped in several areas of the outdoor wooden fence - the tiles, in the bathroom, have black marks and chipped along the bottom area. - in the bathroom, between two sinks, a tile is missing from the wall - one tap is broken from one of the sink. The Manager said that they have informed the directors.
Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.
11020 - CCLR s.13(2) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (f) any medication administered to the child, including the amount and the time at which the medication was administered.
Observation. Upon a review of the minor incident log book, Licensing observed that a child was administered their required medication identified in their care plan. However, there was no record showing the amount and time that the medication was administered to the child. Licensing provided education on importance of ensuring that the medication form is completed whenever a child is given any medication.
Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.
19120 - CCLR s.57(2)(f) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of the attendance records for the month of October 2025, Licensing observed that children are not identified as absent. Licensing reviewed and provided clarification on this regulation and how to complete the attendance records accurately.
Corrective action. The Manager said that moving forward they will ensure that children are identified as absent.
19090 - CCLR s.57(2)(c) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Upon a review of three staff records, Licensing observed that three staff's criminal record checks (CRC) have not been obtained
Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.

Observation. Licensing observed a tall shelf in the eating area that is not secured to the wall.
Corrective action. A written response to be submitted to Licensing on how this contravention has been addressed.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - the toilet paper holder is broken. The toilet paper is kept on the shelf and staff provide some to the children when they need it. - the hand holding bar by one of the toilet has rust all over it.
Corrective action. A written response to be submitted to Licensing on how this contravention has been addressed.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. A sharp knife in the kitchen drying rack that is accessible to children in care.
Corrective action. A written response to be submitted to Licensing on how this contravention has been addressed.
11200 - CCLR s.17 - LICENSING
Licensees must not make any structural change to a community care facility unless the licensee first (b) receives written approval from the medical health officer.
Observation. Licensing observed that a second toilet has been added to the bathroom and a floor to ceiling wall has been constructed between units 3 and 4. Licensing was not made aware of these structural changes. Licensing spoke to the Director,via telephone call at the time of the inspection, and was informed that the changes were recently done within a month. The Director was unaware that they needed to inform Licensing of these changes. Licensing discussed when it is required to inform Licensing and what is required moving forward.
Corrective action. The Director will submit new floor plans with all the changes to Licensing.
10030 - CCLR s.10(2)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of 7 days of the attendance records for the month of October 2024, Licensing observed that children that are absent are not identified as absent and left blank. Licensing had a discussion with the Manager about attendance records.
Corrective action. The Manager will ensure to complete attendance records accurately on a daily basis.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. There is a noticeable musty smell within the facility. Recently a pipe burst within the tenant's suite next door, there is visible water damage within the laminate flooring as it has started to bubble in some areas. It is suspected that the smell may be coming from water damage under the flooring.
Corrective action. Manager stated that they are working on the situation. Please provide Licensing with this plan by November 27, 2023

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following was noted at the time of inspection: -Areas within the facility where the paint has started to chip, in particular within the dining area and nap area. -Some spots where the walls appear dirty within the bathroom and kitchen -Visible dents within the drywall (bathroom & kitchen area) -Carpet near the cubbies in the back appears to be quite soiled.
Corrective action. Please ensure facility is clean and in good repair.

Observation. In the outdoor play space, asphalt is exposed in some areas around a play structure where there should be a thicker layer of wood chips to allow for a resilient surface.
Corrective action. Please ensure a healthy and safe environment.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. -One play table top appears to be in disrepair as some of the top layer of the table was ripped off, exposing a surface that is porous and not easy to clean. -Stepping stool in bathroom is in need of a cleaning.
Corrective action. Please ensure equipment is clean and easy to clean.
11020 - CCLR s.13(2) - HYGIENE AND COMMUNICABLE DISEASE
A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.
Observation. Garbage can used for soiled diapers does not have a cover.
Corrective action. Please ensure that the garbage used for soiled diapers has a cover.
15010 - CCLR s.14(4)(a) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.
Observation. Upon review of attendance, it was noted that on multiple dates (Jane 25,26 & 31), there were more than 2 children under 29 months in care. Staff indicated that some of these children were going to start the sister daycare in the same building in the coming days.
Corrective action. Please ensure that Schedule E is followed.
14020 - CCLR s.34(2)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Licensing could not adequately review attendance for the facility. LO noted that attendance is incomplete when reviewing paper copies of daily attendance. Children are usually signed in but some times the sign out is not written down. Staff indicated that they use an app for attendance. LO requested to see previous attendance for other days but staff did not know how to retrieve this information within the app.
Corrective action. Please ensure that daily attendance is complete and accurate.
19090 - CCLR s.57(2)(c) - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (c) produce records, on demand, to the medical health officer.
Observation. At today's inspection, staff records and full children's records were not able to be accessed.
Corrective action. Please ensure that staff files and children's files are easily accessible. Staff checklists indicating what is within a staff's file are accepted as a method to confirm compliance if there is a concern about leaving staff files unlocked.

- EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.
Observation. The following were noted at today's inspection: -Table lined in felt is not easy to clean, discussed their removal -Cardboard bricks which have been taped with duct tape are not considered to be easy to clean, discussed their removal. -Three shelves at the entrance are not fixed to the wall and could be tipping hazards. -Paint peeling on the fence within the outdoor play space.
Corrective action. Please ensure furniture, equipment is sturdy, safe, easy to clean and free from hazards.
11080 - CCLR s.15(1)(a) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Facility has practiced the emergency drill but haven't entered it into their records since 2019.
Corrective action. Please ensure that emergency drills are recorded in order for Licensing to confirm compliance.
13050 - CCLR s.56(1)(c) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. Care plan on file has not been reviewed since 2018. Manager indicated that there has been discussion with the parent about the care plan, however, there was nothing on the care plan to confirm compliance (i.e. updated signature and date).
Corrective action. Please ensure that care plans are reviewed with the parent each year. Please ensure that some sort of notation is available in order for licensing to confirm compliance.
19260 - CCLR s.58(2)(b) - STAFFING
A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].
Observation. Two staff criminal record checks are not site specific. Manager indicated that they have applied for new criminal record checks but they have not yet been obtained. Discussion had with Manager about applying to have the CRC shared if they already have an existing valid criminal record check with another organization.
Corrective action. Please ensure criminal record checks are site specific.
