11220 - CCLR s.23(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following; - chairs stacked up high in the multi-age program. Potential to fall on a child and injure them. - one set of the stairs leading up to the programs are slippery. Potential for children, staff or families to fall and injure themselves. - fridge temperature measured at -2 degrees, in one of the group child care (30 months to SA) programs. - a shelf in the group child care (under 36 months) program not secured.

11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - garbage in the outdoor area - a carpet at the entrance of one of the programs that is ripped and curling - paint chipped and wear and tear on the walls throughout the facility - several mats ripped exposing the foam underlay - several wooden tables chipped in areas - the outdoor equipment are dirty and have green grime on them

11020 - CCLR s.13(2) - LICENSING
When advertising or otherwise offering services to the public, a licensee must identify the care program being offered.
Observation. There is a sign, imprinted on one of the doors, that says that group child care (SA) is being offered. However, the facility is not licensed for group child care (SA).
Corrective action. A corrective action plan to be submitted to Licensing on how this contravention has been addressed.
10060 - CCLR s.11(3) - LICENSING
A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.
Observation. Two staff's certificates not posted anywhere within the facility. Licensing observed both staff working at the community care facility at the time of inspection.

10070 - CCLR s.11(4) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.
Observation. Licensing observed that following; - a medication identified in one child's care plan was not readily accessible to employees. - a medication identified in one child's care plan has expired and thus not readily accessible to employees

16020 - CCLR s.53(1)(b) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed that there is not sufficient or age appropriate supplies in the emergency bag for 72 hours for the number of children in the facility.

13020 - CCLR s.22(1)(b) - PROGRAM
A licensee must provide to children a comprehensive and coordinated program of indoor and outdoor activities that (c) complies with the program standards set out in Schedule G.
Observation. Licensing observed that there are no outdoor toys and supplies in one of the outdoor playgrounds.

Observation. Upon a random review of eight children's records, Licensing observed that emergency contact information was not obtained for one child.

In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation. Licensing observed that two children's care plans had not been updated annually. However, when Licensing was close to leaving the facility the Manager found the care plans in a different area not with all the other records.

19320 - CCLR s.59(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Children are not identified as absent in the multi-age program.
Corrective action. The Manager will ensure that the attendance records are accurate on a daily basis.

11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - fence in one area where the panel has detached from the rest of the fence. - Several sleeping mats in the programs ripped. - tape on the ground in the multi-age classroom that has ripped and has dirt under it - a diaper change mat ripped. - a wooden table in one of the group child care (30 months to SA) has a lose leg and partially detached from the table
Corrective action. A written response to be submitted to Licensing on how the above noted items have been addressed.
11020 - CCLR s.13(2) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Licensing observed that children's beddings were stored on top of one another. Potential for cross contamination and spreading of germs.

15030 - CCLR s.46(1) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed that some of the food and drinks in the emergency has expired and there was not sufficient items in the bag. Licensing explained that there needs to be sufficient food and drink for 72 hours.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of the attendance records for two weeks of the month of January 2025, Licensing observed the following; -five days that children's time of departure was not recorded. - one day that children were not identified as absent. Licensing explained the importance of accurate attendance records as it is part of active supervision.
Corrective action. The Manager will ensure that attendance records are accurate on a daily basis

Observation. In the group child care (under 36 months) program: - There isn't a infant toddler educator (I/T) at all times in the program. - A responsible adult (RA) works in the program as part of ratio.
Corrective action. The ratio of employee to children must be maintained at all times.
14030 - CCLR s.34(2)(b) - A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. Observation. One of the facility staff's early childhood educator (ECE) certificate was not verified by the ECE registry.
Corrective action. All staff certificates must be verified through the ECE registry.

Observation. The gate door, that families use to get to the group child care (under 36 months) entrance door, is broken. Licensing was informed that it almost hurt a child and parent when they used it to enter.
Corrective action. A corrective action plan needs to be submitted to Licensing on how this contravention will be addressed by May 13, 2024

Observation. The following was noted: -Cubbies in multi-age room and bottom of IT program can act as a topple hazard as they are not secured. -Tables which are used for individual work in the morning are stacked one on top of the other in order to make room for nap time and free play in the afternoon. Stacked tables can act as a topple hazard. -Lysol wipes within the Multi-Age room were accessible to children.
Corrective action. Please provide a written response to indicate how you will address the contraventions noted above on or before February 29, 2024

Observation. The following were noted: -Various shelving/cubbies are not secured to the wall: Cubby that separates the entry way to the classroom in left lower GCC 3-5 classroom, shelving unit within the Multi-Age room, large shelving unity on wheels in lower left GCC 3-5 classroom -Areas where fence slats are a entrapment hazard as the gap between the slats is larger than 3.5 inches.
Corrective action. Please ensure a healthy and safe environment.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. -Closet door in Multi-Age room is broken and off its hinges -Vinyl climbing structure in Infant Toddler room has areas where the vinyl has started to crack and peel. Discussion had with Manager about ensuring furniture can be easy to clean, non porous.
Corrective action. Please ensure facility is clean and in good repair.

When advertising or otherwise offering services to the public, a licensee must identify the care program being offered.
Observation. Service type - Preschool has been mentioned multiple times within the handbook for the facility. There is also a sign at the entrance to the Multi-Age program which refers to Preschool. This facility is not Licensed for a Preschool program and should not be advertising as such.
Corrective action. Please ensure facility advertises according to what service types the facility is Licensed for. Please remove any reference to a Preschool program within handbooks and advertising.

The following was noted during the inspection: -Large shelf is not secured at the top as the bracket screw is coming out of the wall. -Multiple sanitizing/cleaning products were accessible to the children - bulk box of Lysol is stored within reach within the GCC 3-5 program, toilet cleaner stored under sink in unlocked cabinet within GCC 3-5 bathroom, spray container of Lysol accessible to children on shelf by back door.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following were noted at today's inspection: -Some areas where paint is chipping on the walls/doors (i.e. Door of IT nap room, and some walls) -Some walls are in need of cleaning as there are visible finger prints on the walls (i.e. Multi-Age room, Infant Toddler rooms) -Top of book shelves in IT room are tacky in places or have exposed mdf which is not easy to clean and sanitize (repeat). -Fencing that faces 144 Street closest to the building has loose slats. -Yellow fencing that was placed in between the two play spaces due to entrapment hazard has disintegrated, leaving the large gaps exposed.
Corrective action. Please ensure facility is clean and in good repair.

Observation. A care plan does not include a detailed course of action recommended for a child requiring additional support (i.e. after administering medication, whether they seek medical attention, contact the parent, etc). Care plan was also not dated, therefore, LO could not confirm compliance as to whether the care plan had been reviewed yearly.
Corrective action. Please ensure care plans are complete with a detailed course of action and dated.

Observation. The following were noted today: -Various areas within the outdoor play space where the underlay is exposed and can act as a tripping hazard. -A bracket that was used to secure a shelf appears to have come undone from the wall within he GCC 3-5 program. -Toilet brush is accessible within the infant toddler washroom.

Observation. The following were noted at today's inspection: -Multiple areas within the facility that need cleaning, in particular the walls within the Infant Toddler program, Multi-age program (bathroom door and walls), -Multiple areas where the paint is starting to chip on the walls and baseboard - i.e. Infant Toddler program, Multi-Age, bathrooms in GCC 3-5 programs. -Multiple shelving units have a sticky film in certain areas that does not allow for the shelving to be cleaned properly (noted in Infant Toddler program and Multi-age). -One shelving unit in infant toddler program has some trim missing which exposes the mdf, which isn't easy to clean as it is a porous material.
Corrective action. Please ensure facility is clean and in good repair.

Observation. Emergency drill for the infant toddler program was not logged, however, Manager confirmed that it was carried out.
Corrective action. Please ensure a record respecting compliance of emergency drills is kept.

Observation. 7 children's files were reviewed, one child was missing Dr information. A discussion about the acceptable practice of adding a walk in clinic they may attend. Regularly checking with parents to ensure that file information is up to date.
