Observation. Licensing observed a foam mat, used for gross motor activities, is ripped at the corners.
Corrective action. A written response to be submitted to Licensing on how this contravention will be addressed.

Observation. One wood table and wood shelf that have seen some wear and tear over time and the paint that seals the wood making it non-porous has worn off. This makes the table and shelf difficult to clean.
Corrective action. Please ensure a facility in good repair.
