    Observation. Ceiling tiles in washroom have not been replaced leaving toilets in program inaccessible to children.

    Observation. Cubby shelf had been moved and anchored to wall for safety. Faucets in children's sinks were repaired and in good working order. Staff informed Licensing that the plumbing in ceiling is in the process of repair but ceiling tiles have not been replaced so children's toilets continue to be unusable.

    Observation. Landscaped mound with slide in toddler playground continues to show increased erosion creating a steep incline. The steep landscape feature was noted on previous inspection.

    Design: Safety zones are provided under and around equipment to allow for the safe and proper use of the equipment, with adequate space for entering and exiting. [Director of Licensing Standard of Practice - Safe Play Space]

    Observation. The curb of the concrete path at the base of landscaped mound in play yard poses a safety hazard to children playing. Carpet and foam squares were placed on the curb in an effort to prevent injury; however, a wooden round approximately 3 inches thick used to hold foam squares in place presents an additional hazard.
