  Observation. Licensing reviewed four staff files and observed two staff files missing record of the persons immunization status declaration and/or immunization records.

  Corrective action. Ensure not to employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: evidence that the person has complied with the Province’s immunization and tuberculosis control programs.

  Observation. Licensing reviewed two volunteer files and observed one file missing record of the persons immunization status declaration and/or immunization records.

  Observation. Licensing observed no record of a yearly emergency (earthquake) drill and Licesning observed staff state they had not implemented a yearly drill while children were in care.

  Corrective action. Ensure that each employee practices implementing the emergency plan at least once each year.

  Observation. Licensing observed no record of daily outdoor play space checks for 2025; Licensing observed staff state daily outdoor safety/hazard checks are implemented but that staff had not documented the daily checks.

  Corrective action. Ensure to keep a log of all inspections, maintenance and repairs.

• A licensee must not employ a person in a community care facility as an educator or an assistant unless the person holds a valid certificate issued under Division 2 [Employee Qualifications], which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry

  Observation. Licensing observed three staff files had not included a copy of the educator's certificate verification/validation.

  Corrective action. Ensure not to employ a person in a community care facility as an educator or an assistant unless the person holds a valid certificate issued under Division 2 and the Licensee has verified certification.

  Observation. Licensing reviewed five of five children's files. Licensing observed there was no record of photos to readily identify children in an emergency. Staff stated that digital photos were taken but the digital files were overwritten on the camera.

  Corrective action. Ensure that, there is a photo of a child that be used to readily identify child in an emergency.

  3070 - CCLR 57(2)(i)
• A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

  Observation. Licensing observed that there was no record indicating that the two minute flushes for the facility corrective action plan regarding lead in water were conducted. Staff stated that flushes are conducted daily however were not documented.

  Corrective action. Ensure that lead in water corrective action plan is followed and corrective actions are documented.

• In respect of a record referred to in this Division, a licensee must (c) produce records, on demand, to the medical health officer.

  Observation. During continuation of Routine Inspection on March 9, 2023, Licensing did not have access to historical fire drill, emergency drills, and a staff file for a responsible adult.

  Corrective action. Ensure to be able to produce records on demand to MHO.
