Observation. Licensing noted the rubber seal around the washing machine located in the children's bathroom had grime build up and a mold smell was present. The manager stated that the washing machine is used for laundering children's bedding. A second washing machine was available in a different room that staff can access it. This washing machine was clean.

Observation. LO noted a hole in the asphalt on the ramp towards the slide. Issue represents a hazard for tripping.

Observation. LO noted debris was accessible to children. The manager and the licensing officer walked together thought the playground to review the issue. The outdoor check list was marked for the day as the playground was inspected however,debris was not removed.
