    Observation. The potty seat had a strip of duct tape on surface where the children would sit on it. The art storage room of the main playroom did not appear to have a secure latch The outdoor storage shelves at the top of the stairs were not secure There were areas with large amounts of tape making it difficult to clean and sanitize
