    Observation. Licensing observed that the outdoor play space maintenance checklist had not been completed for all of January 2022, 5 times in April 2021, 9 times in May 2021, 5 times in October 2021 and once in December 2021.

    Corrective action. Ensure a log kept of all inspections, maintenance & repairs.
