- A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Observation. Regarding intake March 4, 2026. Through the investigation process, Licensing determined that three incidents occurred that met the definition of reportable incidents under Schedule H of the Child Care Licensing Regulation and were not reported to Licensing.
Corrective action. Ensure that reportable incidents aligning with Schedule H are reported to Licensing within 24 hours. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On April 28, 2026 Licensee submitted a corrective action plan to Licensing. The corrective action plan included: staff training related to reporting and Schedule H will take place through PacificCare and followed up on individually by the supervisor and Manager by June 6, 2026 . The corrective action plan was reviewed and accepted by Licensing.

Observation. Regarding intake March 4, 2026. Through the investigation process, Licensing determined that two children were released to an individual who was not on the authorized pickup list.
Corrective action. Ensure that the Safe Release policy is being followed at all times. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On April 28, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: Confirmation of authorized pick up list with each family and further policy review by June 6, 2026. The corrective action plan was reviewed and accepted by Licensing.
3020 - CCLR 56(2) - Without limiting subsection (1), a licensee must ensure that the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.
Observation. Regarding intake March 4, 2026 Through the investigation process, Licensing determined that based on the staffing schedule provided for October 2025 to February 2026, there were multiple days a week where no staff onsite met the requirements of Schedule E.
Corrective action. Ensure that staff qualifications align with Schedule E at all times. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On April 28, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: ongoing and day-to-day monitoring of staff qualifications and ratios. The corrective action plan was reviewed and accepted by Licensing.
