Observation. Related to intake dated January 27, 2026. Through the investigation process, Licensing determined two children arrived at the same time, but were signed in two hours apart on the attendance app, Kindertales.
Corrective action. Ensure children's records show time of arrival and time of departure. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On February 9, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the Licensee will implement paper attendance records in addition to Kindertales attendance records to ensure consistency and accuracy. This corrective action plan was reviewed and accepted by Licensing.

Observation. Wood chip depth surfacing under the five foot (1.52 m) climbing dome measured 2 to 4 inches (8 probes).
Corrective action. Ensure elevated equipment of five feet (1.52 m) has a minimum of six inches of depth surfacing.
