Observation. The facility has not been practicing the emergency evacuation plan as per the center policy.
Corrective action. Ensure an Emergency Evacuation policy is developed and followed.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. - The grass in the playground has not been maintained and present tripping hazards. The grass area have several spots where the grass has been turned up and present a tripping hazard. - Broken toys and standing water collecting in broken toys and equipment was observed - the climber is showing signs of wear, such as chipped paint and areas that could produce splinters.
Corrective action. - Ensure the grass in the playground is maintained and all potholes are filled when possible. - Ensure there is a system in place to check for broken toys each day and disposed of. - Ensure that staff are inspecting the playground for standing water and removing such things prior to children playing outside -Playground climber requires maintenance to ensure it is in good repair. A system in place to ensure the climber is in good repair at all times.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. In the outside play area the following was observed: - there are holes or openings in the fencing in 2 different places that present a large enough gap for a child to squeeze under or become entrapped. - Feces were observed in the playground area near the fence on a cement barrier at child level. - Mushrooms are growing throughout the playground area - Both Geo climbing domes located in the play yard do not have sufficient clearance from hard objects located close by.
Corrective action. - Areas of entrapment need to be secured - A system in place to remove toxic substances and feces must be in place before children enter the playground

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. The nap beds were made with the individual children's sheets and blankets and then stacked together with the sheets and blankets touching
Corrective action. Ensure the children's individual sheets and blankets are stored separately

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Food and water in the emergency supply bin was not sufficient
Corrective action. Enough food and water for approximately 72 hours in care for each staff and child. Food supplied in the emergency supplies should not be candy.

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.
Observation. A care plan does not include the course of action recommended by health care professionals for a child requiring additional support.
Corrective action. The manager stated that a child with allergies had an epi pen but the parents removed the epi pen from the facility because it expired. The family asked the facility to call them if the child has a reaction this is not recorded in the child's care plan

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The following was observed in the outdoor play area: 1. The climbing structure that was installed a few years ago has parts that are not secured to the ground and can be moved easily by either climbing or pushing on the structure. This structure has been installed directly next to a concrete surface. 2. The outdoor sandbox no longer has a functioning cover. The sand has eroded and no longer provides an adequate amount of sand for play. 3. The Geodome Climber has been installed directly next to a concrete surface.
Corrective action. 1. Ensure the climbing structure is secured and that there is a six foot clearance in all directions from the concrete surface and other equipment. 2. Ensure the sandbox has adequate amount of sand for play and a cover. 3. Ensure the Geodome has a six foot clearance in all directions from the concrete surface and other equipment

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following in the outdoor play area: - A Geodome does not have sufficient resilient surfacing around it. The manager stated the landscaper moved the geodome from the grass area to the wood chips when they were cutting the grass. - A wooden indoor play kitchen is stored in the unused sandbox area. The kitchen is starting to fall apart and mold is growing on the back.
Corrective action. Ensure a healthy and safe environment is provided at all times.

Observation. Licensing noted the name of the new appointed manager is not displayed.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.
Observation. A selection of staff files were reviewed. Out of the 3 files reviewed, immunization status was not obtained for one employee.
Corrective action. Ensure there is evidence that employees have complied with the Province's immunization and tuberculosis program.

Observation. Licensing observed the following inside the facility: - Children's chairs that are soiled - There is a TV stored on top of a high shelf posing a risk to the children if it were to fall. - Plastic bags were stored under the sink in the children's bathroom Licensing observed the following in the outdoor play area: - There is large cement and metal bolders stored in the playground area that is accessible to the children. This could pose as a tripping hazard for the children. - There are Little Tikes climbers in the playground area. There is insufficient resilient surfacing around the equipment.
Corrective action. Please ensure a healthy and safe environment is provided at all times.

Observation. Licensing observed the following inside the facility: - Expired emergency supplies. - The fire extinguisher was not visible as there was binders and a storage bin in front of it. (CORRECTED DURING INSPECTION) - The pull station is not visible as it is covered with a piece of paper.
