  Observation. Licensing audited five staff files. Two staff were missing their immunization records.

  Corrective action. Ensure that the immunization status was on file for each staff.

  Observation. Licensing audited ten children's files. One child was missing their doctor's name and phone number.

  Corrective action. Ensure that doctor's name and phone number was on file for each child.

  Observation. Licensing observed cleaning supplies were not out of children's reach and a toy shelf was not mounted to the wall tightly.

  Observation. Licensing observed the play equipment were not six feet apart.

  Corrective action. Ensure the play equipment were 6 feet apart.

• A licensee must not employ a person in a community care facility as an educator or an assistant unless the person holds a valid certificate issued under Division 2 [Employee Qualifications], which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry

  Observation. Observation: Licensing audited six staff's files. Two staff's Early Childhood Educator's certificate validations were expired.

  Corrective action. Ensure staff's Early Childhood Educator's certificates were validated.

  Observation. Licensing observed no care plan was developed in consultation with parents and any person requested by the parent for a child who requires medication.
