  Observation. Licensing reviewed five of thirty children's records. Two children's records were missing photos.

  Observation. Licensing reviewed five of thirty children's record. One children's record did not have a copy of the repayment agreement signed by the child's parent/guardian.

  Corrective action. Ensure that, for each child, there is a copy of the signed repayment agreement.

  Observation. Licensing reviewed documentation of two minute flush for lead in water corrective action plan. Licensing observed that two minute flushes were last documented in November 2022. Manager stated that two minute flushes were conducted daily upon arrival to the facility.

  Corrective action. Ensure that corrective action plan for lead in water is followed and documented.
