- A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.
Observation. Licensing reviewed five children's records. Two children did not have MSP numbers on file and two children had only verbal confirmation of immunizations status with no written records on file of immunizations status.
Corrective action. Ensure all children have MSP and immunization status on file.
