- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees. Observation (CORRECTED DURING INSPECTION): Licensing observed the following: - Inside the 3-5 bathroom, bath mats were observed on the floor in front of the sinks. Staff said they were used for children to sit on when changing into/out of a pull up. As these mats are on the floor, other children are walking on them from using the toilet and there is concern for cross contamination. - Outside there are various weeds encroaching into the playspace and accessible to children - regular maintenance is not being addressed. THIS WAS NOT CORRECTED ON SITE
Corrective action. Staff removed the mats during the inspection. Please ensure these mats are single use and only placed when required.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following in the outdoor play spaces: - Uneven surfaces due to lifted rubber tiles and sunken areas of sand - Chipped/broken hardy board siding in areas accessible to children - Crumbling concrete which has caused trips and falls
Corrective action. This is a repeat contravention. Staff said that some kind of plan is in the works to address these issues. Please provide a compliance plan to Licensing, outlining mitigating the risk of harm to children, by the date below
11020 - CCLR s.13(2) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. Licensing observed three care plans posted that had not been reviewed annually with the parent or any person requested to by the parent.
Corrective action. Please ensure all care plans are current and accurate by reviewing with the families.
19260 - CCLR s.58(2)(b) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person. Observation (CORRECTED DURING INSPECTION): On review of the staff checklist available on site, Licensing observed two staff currenly working had expired Criminal Record Checks. These staff should not be supervising children until a current clearance is available. Area Manager said that the dates on the checklist are not accurate only to YMCA requirements. Current clearances are in place for these staff.
Corrective action. Please ensure records are accurate for review

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. During a walkthrough of the outdoor play spaces for both service types it was noted that the following substance(s) were present and are hazardous to children in care: -overgrown nut trees which could cause allergic reaction -Norway maple tree accessible to children which can be toxic to children -unidentified weeds/stalks of foliage that could cause harm to children in care
Corrective action. Licensee rep states that landscaping will take place to ensure above items are made inaccessible to children in care
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. During a walk through of the outdoor space on the I/T side noted: -equipment placed over concrete areas that could causing a trip/fall as a temporary solution to prevent injury
Corrective action. Licensee states a plan will be developed and implemented in order to address concerns

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. A walk through of both outdoor play spaces took place, and revealed the following equipment is in disrepair: -wood on play house rough, and could cause splinters -black tile lifting causing a tripping hazard -sandy area in 3/5 place under maple tree is sinking causing a tripping/fall hazard -hardy board/building trim on north side of building is broken with jagged edges accessible to children as it is next to the sandbox
Corrective action. Licensee states a plan will be developed and implemented in order to address the play space concerns

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. A walk through of the outdoor play space revealed the following equipment is in disrepair: -tile on 3/5 playground lifting and causing a tripping hazard -wood on play house is showing signs of rot -mast on 'boat' showing signs of rot, and is not secure -concrete around edge of rubber mats at north and south ends of playground is lifting presenting as a trip hazard -large pools of standing water are forming on the rubber mat -hardy board around the bottom edge of the northwest corner of the outside deck is broken and has jagged edges -storage shed is showing signs of rot, nails exposed -wood trim around edge of sandbox is showing signs of rot
Corrective action. Please ensure facility is in good repair to ensure the health, safety and dignity of the children in care
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. During a walkthrough of the outdoor space the following was noted: -sloped stair on the infant/toddler stairs leading from the upper deck to the ground which could cause a child to fall -gaps on wood where small fingers could be pinched or received splinters -equipment placed over top of hazards as a temporary solution to prevent children from accessing unhealthy/unsafe products within the play space
Corrective action. Please ensure the outdoor environment is healthy and safe for all children in care

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. During a walk through of the outdoor play space the following substance were observed that may be hazardous to children in care: -asphalt around the rubber matting on the infant/toddler side of the playground is crumbling and is small enough to be ingested -nut trees overgrown which could cause potential allergic reaction -Norway maple tree accessible to children which can be toxic to children
Corrective action. Ensure all hazardous items/substances are removed or repaired

10050 - CCLR s.11(1) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (h) any special instruction respecting the child's diet, medication, participation in a program of activities, or other matter relevant to the child's care, (i) given by the child's parent to the licensee in writing.
Observation. Review of children's files revealed one file indicating a medical condition, however staff were unable to located documentation of the medical condition. Staff were able to explain two symptoms of the condition, however are unsure if the condition is even relevant any longer
Corrective action. Please ensure all medical condition information is readily available to all staff or if no longer relevant remove the indication
